Frequently Asked Questions

Q: WHERE DO I SEND MY PAPERWORK? 
The Immunization Form goes to the Student Health & Wellness (SHW) 
All other forms should be uploaded into or electronically signed in Heatlthy Roster.
  
Q: HOW DO I ACCESS SHW Medicat? 
You can access directions for all that is required through SHW by following this link: Health and Wellness Center Immunization and Medicat Information 
 
Q: HOW DO I ACCESS Healthy Roster? 
A: You will find all instructions on how to access and upload forms on Healthy Roster

Q: WHAT IS THE DUE DATE FOR SUBMITTING MY PAPERWORK? 
New incoming freshman and transfers:  
June 30: Immunization form Medical History Questionnaire through SHW online Medicat Portal 
July 1: Physicals (done within 6 months of traditional/non-traditional practice start date) & Physician packet on HealthyRoster
 
Q: DO I NEED TO HAVE MY PHYSICIAN FILL OUT THE ADD/ADHD FORM IF I DO NOT HAVE IT? 
No, if you do not have an official diagnosis of ADD/ADHD, you do not need to have your physician complete this form. 
  
Q: I THINK I HAVE ADHD AND MY DOCTOR SAYS I DO, BUT I WAS NEVER FORMALLY TESTED.  CAN I STILL TAKE THE MEDICATIONS AND HAVE MY PHYSICIAN FILL OUT THE FORM?  WILL I BE OK DURING DRUG TESTING?  
No, you cannot take medications without formal testing for ADD or ADHS. The NCAA requires proof of testing for ADD or ADHD. Your formal testing must be on file within the Sports Medicine Department for you to be allowed to take medication for ADD or ADHD in accordance with NCAA guidelines. 
  
Q: I DON’T THINK I HAVE SICKLE CELL.  CAN I JUST SIGN THE WAIVER?  
NO! The NCAA now requires proof of sickle cell testing.  You will need to contact your primary care provider to obtain a physical copy of your sickle cell testing, subsequent results and upload them to Healthy Roster.  If you have never been tested for sickle cell, you will need to contact your primary car provider to schedule sickle cell testing.  The cost of sickle cell testing will not be covered by the college.
  
Q: I CURRENTLY TAKE SUPPLEMENTS AND MEDICATIONS.  WHERE CAN I FIND A LIST OF NCAA BANNED SUBSTANCES? 
You can find the list of NCAA banned substances HERE. You need to provide a list of any medications that you are currently prescribed to the Sports Medicine Staff within a note from your physician stating why you need to take the medications and the dosage amount.  
 
You can also go on Drug Free Sport and login.

You can look up any medication or can send a request for Drug Free Sport to look at a specific supplement and email you back whether it contains a banned substance.  
 
*NOTE: Any supplement that you take is at your own risk even if Drug Free Sport does not say that it contains any banned substance as supplements are under regulated by the FDA* 
  
Q: CAN I USE A COPY OF MY INSURANCE CARD FROM LAST YEAR?  
No, you cannot. We must have a copy of your insurance card every year to ensure all information is current. 
  
Q: WHAT IF I HAVE STUDENT HEALTH INSURANCE THROUGH SUNY ONEONTA?  
You will need to indicate in Healthy Roster under the Insurance tab that you have student health insurance. We will still need a copy of your card once you receive it from the school. 
  
Q: DO I HAVE TO COMPLETE THE COLLEGE ALCOHOL EDUCATION PROGRAM? A: All Incoming Freshman and Transfer Student-Athletes under the age of 21 must complete the college’s Alcohol Education Program to be eligible to participate in athletics. 
  
Q: DO I NEED MY PARENT’S SINGATURE IF I AM OVER 18?  
Yes, you do need your parent’s signature, but only for the Insurance Acknowledgement Form. If you are over the age of 18 and still on your parent’s insurance, we need their signature to give us permission to use their insurance.  They must acknowledge that there is no Secondary Insurance through SUNY Oneonta and all medical expenses accrued are the responsibility of the student-athlete and/or their parent/guardian.
